Processes discharged records and ensures completeness of the record at discharge.
Retrieves discharge charts from the clinical units and conducts discharge reconciliation steps against the discharge report.
Assembles the record in accordance with policy and procedures. Coordinates re-distribution of chart binders to the units. Makes discharged records available for scanning process.
Analyzes medical records for quantitative completeness based on indicated standards and departmental policies. Correctly flags deficiencies.
Notifies responsibly party of deficiencies in record completion and assists in completing deficiencies in a timely manner.
Assembles patient records timely and accurately which includes prepping, scanning, and indexing documents.
Processes medical record requests and validates the appropriate access of medical record to the requestor. For paper records, retrieves and checks record to the appropriate location on chart management system. For electronic records, manages the provision and expiration of the electronic record access for the requestor.
Maintains knowledge of all applicable laws and regulations for completion of the legal medical record. Adheres to department compliance for privacy and security of patient information (i.e. HIPAA and HITECH).
Performs validation checks to ensure all records are accounted for and complete. Analyzes documents and assigns deficiencies for physician medical record completion for all patient types. Interacts openly and honestly with physicians, other healthcare professionals, patients and the public. Understands and explains complex electronic completion activities to physicians. Demonstrates behavior and conduct that is consistent with Via Christi's philosophy, policies, procedures and standards.
Minimum Qualification: None Required
HS or Equivalent
Prefer Associate's degree and/or RHIT certification and/or 2-4 years related experience/training in health information management
