Job Description

Job Summary:

Applies the appropriate diagnostic and procedural code to patient health records for purposes of document retrieval, analysis and claim processing.

Responsibilities:

  • Abstracts pertinent information from patient records. Assigns the International Classification of Diseases, Clinical Modification (ICD), Current Procedural Terminology (CPT) or Healthcare Common Procedure Coding System (HCPCS) codes, creating Ambulatory Patient Classification (APC) or Diagnosis-Related Group (DRG) assignments.

  • Obtains acceptable productivity/quality rates as defined per coding policy.

  • Queries physicians when code assignments are not straightforward or documentation in the record is inadequate, ambiguous, or unclear for coding purposes.

  • Keeps abreast of and complies with coding guidelines and reimbursement reporting requirements.

Qualifications

Licenses/Certifications/Registration:

  • Required Credential(s):

  • Drivers License credentialed from the Washington Driver Licenses obtained prior to hire date or job transfer date. Must have/obtain current Liability Insurance to use personal or LHN vehicle for business purposes.

  • One or more of the following:

  • Certified Coding Associate (CCA) required within 90 days of hire. When associate obtains their CCS, CCS-P or RHIT the CCA is no longer required.Certified Coding Specialist, CCS-P or RHIT required within two (2) years of employment.Preferred certifications: CASCC - Ambulatory Surgical Center; CEDC - Emergency Department; CEMC-Evaluation and Management, CGIC-Gastroenterology, CGSC-General; COSC-Orthopedic Surgery; CENTC-Otolaryngology, CPEDC-Pediatrics and CUC-Urology.

  • Certified Coding Associate (CCA) required within 90 days of hire. When associate obtains their CCS, CCS-P, Certified Professional Coder or RHIT the CCA is no longer required.Certified Coding Specialist, CCS-P or RHIT required within two (2) years of employment.Preferred certifications: CASCC - Ambulatory Surgical Center; CEDC - Emergency Department; CEMC-Evaluation and Management, CGIC-Gastroenterology, CGSC-General; COSC-Orthopedic Surgery; CENTC-Otolaryngology, CPEDC-Pediatrics and CUC-Urology, and COC-Certified Outpatient Coder

  • Professional Coder credentialed from the American Academy of Professional Coders (AAPC) obtained within 3 Months (90 days) of hire date or job transfer date. Certified Coding Associate or Certified Professional Coder required within 90 days of hire.

  • Certified Coding Associate (CCA) required within 90 days of hire. When associate obtains their CCS, CCS-P, Certified Professional Coder or RHIT the CCA is no longer required.Certified Coding Specialist, CCS-P or RHIT required within two (2) years of employment.
