Social Worker investigates, assesses and plans interventions to help patients cope with social, emotional, economic and environmental problems.
Interviews patients, assesses priorities and documents case activity.
Consults with team managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan.
Assists clients/families with obtaining community assistance by referral to proper resources.
Develops and maintains working relationships with community health, welfare and social agencies and seeks creative means to assist the client with his/her needs.
Advocates for the needs, decisions, and rights of the patients and families in palliative and end of life care.
Reviews and adjusts care delivery according to the changing care needs throughout the course of illness and into bereavement.
Engage in a collaborative process of decision making that involves the patient and family and Palliative Care team to establish relevant and feasible care plans.
Provide information to patients and families accordingly at the end of life.
Licensed Clinical Social Worker credentialed from the Florida Department of Health obtained prior to hire date or job transfer date.
Master's Level Degree
- 3+ years experience in related field required.
