Job Description

Job Summary:

The Residential Case Manager investigates, assesses, and plans interventions to help patients cope with social, emotional, economic and environmental problems.

Responsibilities:

  • Interviews patients, assesses priorities and documents case activity.

  • Consults with team managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan.

  • Assists clients/families with obtaining community assistance by referral to proper resources.

  • Develops and maintains working relationships with community health, welfare and social agencies and seeks creative means to assist the client with his/her needs.

  • Develops, implements and monitors individualized Residential Program Plans.

  • Responds to critical/emergency residential incidents on a 24 hour on call basis as scheduled.

  • Assists residents with person care needs and provides hands on assistance, including coaching and teaching activities of daily living.

  • Provides transportation to scheduled appointments/activities as needed.

  • Completes initial and on-going functional assessments and progress notes including coordination with family and community providers.

  • Other Duties as assigned

Qualifications

Education and Work Experience:

  • Bachelor's Degree REQUIRED

  • Bachelors Degree Human Services, Social Work, Psychology or Related Field (preferred)

  • Equivalent combination of Education and Experience in Human Services, Social Work, Psychology or Related Field considered

  • Previous experience in working with Community Based Programs for persons with serious Psychiatric and Addiction Disorders helpful (preferred)

  • Experience in working with the homeless helpful
