Coordinates the overall interdisciplinary plan of care for patient, from admission to discharge.
Creates plan for care across the continuum, integrating patient/family preferences and values.
Monitors patient care through assessments, evaluations and/or patient records.
Advocates for resources and removal of barriers.
Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
One or more of the following:
Licensed Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date.
Licensed Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained within 12 Months (1 year) of hire date or job transfer date. (LSW or LCSW)
Bachelor's Level Degree
Must either be licensed RN (Bachelor's degree in Nursing preferred) or licensed Social Worker (Master's degree in Social Work required)
Three years of experience in case management and/or clinical nursing preferred.
RN/BSN required or MSW or MSSA w/ LSW (required within first 12 months)- LCSW preferred
